The application process

One request, four clear steps.

You can save your information, submit when it is ready, and return to your dashboard to see updates.

  1. Create your account

    Use email and password, Google, or Apple. Returning applicants can sign in instead.

  2. Complete your profile

    Share your contact information and service background so the team can follow up.

  3. Describe the need

    Tell us what equipment or repair is needed, why it matters, and how urgent it is.

  4. Submit for review

    The foundation reviews the request and contacts you if more information is needed.

What to prepare

A little detail helps us understand the need.

Start with the information you have. The team may request additional documentation during review, but you do not need to upload medical records on the public website.

Review time depends on the request.

The foundation does not promise a fixed approval or delivery timeline. Timing depends on urgency, available equipment, funding, and any information needed to verify the request.

Rise & Roll is not an emergency service. If someone is in immediate danger, call 911. For an urgent mobility or equipment safety issue, mark the request as urgent and contact the foundation.
